My Account address edit causes refresh and format issue

I have setup the My Account template and assigned it via WooCommerce Theme Builder. Everything looks great, until, a user goes and edits their Shipping or Billing address.

The page is refreshed, the format is lost and stretch across the screen. In the demo it looks like the edit field is local to that field/element that the element is located.
